Program Description: A health insurance program for Indiana residents who experience problems in obtaining or keeping health insurance due to a medical problem. ICHIA was created by the Indiana legislature to offer residents of the state, through the participation of health insurance companies. The program administrator is Acordia Healthcare Inc.

Eligibility:
Resident of Indiana
Does not have or cannot have other insurance coverage
Has received one rejection notification form a health insurer

Note: If a person suffers from any specific medical condition on the ICHIA list, they do not need rejection notification.
Program Cost: As you enroll in ICHIA, you will be given four health plans to choose from. These plans differ in the amount of the premium, deductible, and coinsurance payment. These plans are described in detail in a brochure from ICHIA. You are also given a choice on the frequency of your payments; monthly, quarterly, semi-annually, or annually. Your rate is based on area, age and sex, and is subject to change. Medicaid is NOT accepted. Medicare is NOT accepted. Private insurance is NOT accepted.

Health insurance program offered by the State of Indiana for children from birth through age 18. It covers check-ups, well-child visits, doctor visits, hospital stays, and prescription drugs as well as vision and dental care. In general, there are no deductibles, co-payments, or premiums in Hoosier Healthwise for Children. There is an eligibility criteria based on family income.

How do I apply?
There is a two-sided one page application. You can apply here at Riley, downstairs on first floor at the Children with Special Needs office. When you apply, make sure you have proof of your family's income, like tax forms or pay stubs (three of them). You will also need your child's Social Security number and in some cases, proof of their immigration status.
